Decoct methods

I am just starting to do all grain brews and have been reading up on it but was wondering if someone could lay the process out for a newb. The last two times I seem to mess up one place or another because I can not find a good explanation of the process. Any help would rock!  Thanks!
 
Here's the basic idea:
  - Rather than adding hot water (infusion mashing), you draw a portion of the existing mash (both water and grain) and boil it, then add that boiling mash back to the existing mash to reach the next mash step.
  - BeerSmith will calculate how much mash you need to draw off and boil - just use any of the decoction profiles and follow the instructions on the brewsheet.
