I'm thinking of making the jump to BIAB, having brewed extract for a few years now. What is a tried and true way to convert a 5 gallon all-grain recipe to a 5 gallon one for BIAB?

The best tried and true way is to take your 5-gal all-grain recipe and brew it!  BIAB is all-grain, just without all the fancy equipment and resultant clean up work.  There may be some difference in efficiency due to grain crush, but until you actually brew it, you will not know what adjustment to make.  This adjustment occurs with any all-grain recipe, since each brewer has a slightly different set up and process.

What Oginme said. Or you can set up a new equipment profile and new mash profile. Here are a couple of links to videos that do a good job of explaining both:

Bob357 has hit the nail on the head. You really want to set up customized equipment and mash profiles to get the most out of Beersmith. I have equipment profiles for 5 gallon BIAB system... 5 gallon 3-kettle system and 10 gallons 3-kettle system. btw, I don't have three different brew systems. It's all the same rig, I just brew on it three different ways. Then I have 12 mash profiles. Six for BIAB and six more for my three vessel brewing. All of these profiles set up using the brulosophy tutorial videos on Youtube. It takes time and effort to set these up but it is well worth it.
