Are adjunct mashes always unmalted grain?

are unmalted grains only considered adjunct mash?

also in a wheat beer which generally has around 30% malted wheat would the malted wheat be considered an adjunct as it is not the primary grain but is malted?
 
No not always. While the common definition of an adjunct is an unmalted starch some use the term adjunct for malted wheat or rye. There are also kettle adjuncts like fermentable sugars.
