On beersmith, on recipe design, in low right corner, is measured batch size before or after the % cooling shrinkage rate?  It makes a difference in measured efficiency.  Thanks.

Also, does include trub loss to chiller, or the volume of cooled wort in kettle prior to dumping into carboy?

Neither. Measured batch size is your net yield in the fermenter. It's what's left after all losses prior to pitching and fermentation.

Trapae;

It's pretty common for brewers to take a while to fully understand both BE and how BeerSmith approaches many brewing steps. Maybe some other resources would help?

Here is what Brad has to say on the subject:
http://beersmith.com/blog/2008/10/26/brewhouse-efficiency-for-all-grain-beer-brewing/

Brewer's Friend lays it out in mathematical terms, too:

BeerSmith takes variables like temperature expansion into account and applies them in an "as-you-should-read-it-at-that-moment" form for brewing steps and instructions.

The Braukaiser blog sums up some of the conflict between the BeerSmith definition and what other texts call Brewhouse Efficiency:
