Total efficiency

When I design my recipes with beersmith 2, real values of  gravity only matching when I set the "Total Efficiency" to %42.

Could it be the reason of this ;

1. low quality of Barley
2. complete the germination too early
 
It could also be due to:

Poor conversion of starch (temperature, mash pH)
Poor extraction of starch from grain (coarse crush of grains)
Poor lautering efficiency (poorly drained areas in mash tun, channeling of sparge water)
Too much dead space in your system (lauter tun dead space)
Too much wort losses after mashing (losses to trub and chiller)
