Way too much left over wort

Hello everyone.
I am making a barley wine.

I should have 4.75 gallons in the fermenter.
I hit my boil volume size with 6.85.
My pre-boil OG should have been 1.078, but I got 1.068 with 1 1/2 gallons of wort. I checked the OG of the last 1/2 gallon, it was 1.055. I got my needed numbers by adding 2 cups of DME.

Funny, the brew sheet said "Prepare 9.01 gallons of water." but I was to mash in with 6.77 gallons and sparge with 3.43. Uh, that is 10.2 gallons if you add them up.

Where did I go wrong? I prepared 10 gallons only because of the mash and sparge water.
