AHA standard recipe report form

Does anyone have a custom report for BSmith that creates the standard AHA recipe form that some comps require?    Like the attached PDF?

If so, please post here?  Thanks.

It could probably be done (with some HTML) in the current version - I'll take a close look at it and see if I can come up with a template for the next version.
