Question about plate chillers

If I get one I assume this means I can't leave the cold break behind when I rack to primary or is there a step I am missing.
 
I think you are referring to the cold break material you leave behind in brew kettle.  I use a mesh hop screen that filters/screens out the material before it exits kettle and enters plate chiller.
 
On another but related note. I Could use some direction here. Everything I've been reading on how to control wort temp as it exits the plate chiller is all over the place. So does one control the wort temp by adjusting the water on the input or output side. Or, does one control it by the wort input or output? Help.
