In Zaninasheff's Antwerp BPA he recommends increasing to 90 min boil with all grain option. When do i add first hop addition? At 60 or 90 min?
 
Hop additions still happen at the same time intervals as the extract version. The additional boil time concentrates the wort a little more, creates hot break and gets rid of more DMS.
 
Hop additions are measured from flame-out, so 60 is usually the standard maximum. 

First-wort hops end up being in there the entire boil, whatever that time may be.
