Difference between Gravity After Secondary and Final Gravity

Hi,

I'm trying to find out the difference between Gravity After Secondary and Final Gravity. If final gravity is measured as the beer goes into the bottles or keg then they would seem to be the same. Is Final supposed to be measured after carbonation is complete?
